For your application to be considered, you must be regularly attending the campus you select on your application.

Step 2: Financial Information

What steps are you personally taking to solve your current situation? is required.
Step 3: Monthly Budget Worksheet
Please download and fill out the monthly budget worksheet. This will need to be completed before your application is considered. The worksheet can either be uploaded (in the Uploads section below), emailed, mailed, or dropped off to
Bayside's Care Team.
Step 4: Bill Information
List the bills you need assistance with in order of priority. Please remember we can assist with Cable/Internet, Car Payment, Dental, Rent, Utilities, Car Insurance, Car Repair and Phone bills only. No exceptions. Upload copies according to Bayside's guidelines.
Step 5: Uploads
Please attached the uploads below, they will all need to be provided before your application is considered. If you are unable to upload, they can be emailed, mailed, or dropped off to Bayside's Care Team.
